Benefits at Super-Dog

This is only a summary of the benefits we offer to our employees. When someone joins the Super-Dog Team, they receive detailed information about eligibility, enrollment and other important information.

  • Medical Insurance
    • Two co-pay PPO medical plan options are available through Highmark Blue Shield. Both the Standard and Value PPO plans allow you the freedom to seek care without referrals and offer the same level of medical services; however coverage amounts (deductibles, copays/coinsurance, and out-of-pocket maximums) vary by plan. The Value Plan includes a Health Reimbursement Arrangement.
  • Prescription Drug Coverage
    • Retail pharmacy (31-day supply): Copayments of $15 Generic Drugs, $25 for Brand Named Drugs, and $40 for Non-Formulary Brand Drugs.
    • Maintenance Drugs through mail order (90-day supply): Copayments of $30 Generic Drugs, $50 for Brand Named Drugs, and $80 for Non-Formulary Brand Drugs.
  • Dental Coverage
    • PPO Plan through United Concordia with 100% coverage for Preventive and Diagnostic Services.
    • A $50 individual / $150 family annual deductible applies to Basic and Major Services. Once the deductible is met, members are responsible for 20% of Basic Services and 50% of Major Services.
  • Vision Coverage
    • Exams covered in full and coverage amounts for all other services when utilizing a participating provider through National Vision Administrators. Payment schedule for use of a non-participating provider.
  • Flexible Spending Account (FSA)
    • Medical FSA to defer money on a pre-tax basis to cover eligible medical expenses.
  • Life Insurance and AD&D (Company Provided)
    • Life Insurance of $25,000
    • Accidental Death and Dismemberment - up to an additional $25,000
  • Short Term Disability (Company Provided)
    • Provides wage replacement for up to 26 weeks if absent from work because of an approved non-occupational illness or injury
    • Weekly disability benefit is the lesser of 66 2/3% of your weekly earnings or $1000.
    • Benefits begin on the first day of an injury, or on the 8th day for illness.
  • Voluntary Insurance Options_(Employee-paid at group rates)
    • Long Term Disability
    • Voluntary Supplemental Life Insurance for employee, spouse and children
  • 401(k) Retirement Plan
    • Plan allows employees to contribute up to 50% of their earnings for their retirement needs on a pre-tax basis.
    • Super-Dog will match employee contributions of up to 4% of compensation. The matching contribution will be $.50 for every $1.00 that you contribute.
    • Eligible to participate after three months of service
  • Time Off
    • Super-Dog recognizes the importance of having time away from work. Because of that, we offer a Personal Time Off (PTO) policy in lieu of traditional vacation time, sick time, and personal time. PTO days are intended to be used for vacation time, sick days, birthdays, doctors’ appointments, or for other personal reasons.
    • In addition Super-Dog offers 6 company-paid holidays.
